About the Role:

The Home Health Aide plays a critical role in providing compassionate and personalized care to individuals in their own homes, ensuring their comfort, safety, and well-being. This position involves assisting clients with daily living activities, monitoring their health status, and supporting their independence while respecting their dignity. The aide will work closely with healthcare professionals and family members to implement care plans tailored to each client's unique needs. By delivering attentive and reliable care, the Home Health Aide helps improve the quality of life for clients, particularly those with chronic illnesses, disabilities, or age-related conditions. Ultimately, this role contributes to enabling clients to remain in their homes safely and comfortably for as long as possible.

Minimum Qualifications:

• High school diploma or equivalent.
• Certification as a Home Health Aide (HHA) or equivalent state certification.
• Basic knowledge of patient care and assisted living practices.
• Ability to perform physical tasks required for client care.
• Proficiency in basic computer skills for documentation purposes.

Preferred Qualifications:

• Experience in long-term care or home care settings.
• Training in home settings and healthcare environments.
• Familiarity with electronic health record (EHR) systems.
• Additional certifications such as CPR and First Aid.
• Strong interpersonal skills with a focus on patience and empathy.

Responsibilities:

• Assist clients with personal care tasks such as bathing, dressing, grooming, and toileting to maintain hygiene and comfort.
• Provide support with mobility and transfers to prevent falls and promote physical activity within clients' capabilities.
• Administer medication reminders and monitor clients for any changes in health or behavior, reporting concerns promptly to healthcare providers.
• Perform light housekeeping duties including sanitization to maintain a clean and safe living environment.
• Document care activities and client progress accurately using computer data entry systems while maintaining confidentiality.
• Work independently with minimal supervision, managing time effectively to prioritize client needs and appointments.
• Communicate empathetically and respectfully with clients and their families to foster trust and emotional support.
