Nurse Care Manager – Managed Long-Term Care (MLTC)

Location: Queens, NY (Hybrid: 4 days remote, 1 day field after onsite training)

Sign-On Bonus: $20,000 ($5,000 payable every six months, less taxes)

Key Responsibilities:

  • Conduct in-home assessments and develop personalized care plans for members.
  • Collaborate with members, families, caregivers, primary care practitioners, and interdisciplinary teams to ensure consistent, high-quality care.
  • Authorize covered services and coordinate care across different payers.
  • Monitor members’ needs and health status to maintain the most independent living situation possible.
  • Advocate for members while promoting quality and cost-effective outcomes.

Qualifications:

  • Current NYS RN License and registration.
  • Associate's degree in nursing required; Bachelor of Science in Nursing (BSN) preferred.
  • Minimum of 3 years of RN experience.
  • Clinical expertise in geriatrics preferred; experience in managed long-term care required.
  • Bilingual in Chinese, Cantonese, or Mandarin required.

Compensation & Benefits:

  • Annual salary: $105,000 – $115,000.
  • Eligible for a $20,000 sign-on bonus ($5,000 payable every six months, less taxes).
